- Please submit an application form at least four days prior to when you want to be treated
- Please check your policy to ensure your illness / disability is covered
- Please check that your policy covers the treatment fees requested
- Before we organise any cashless medical treatment, we’ll need to check your account to ensure there are no outstanding monies owed to Bolttech so we advise that you settle any outstanding bill(s) in advance
