Before treatment
Check if your policy covers the claim that you are eligible to make.

Log in to your FWD eServices account via:
- FWD eServices mobile app
- FWD eServices web portal
- Wechat FWD HK official account.

Download the correct claim form

After treatment
Collect the following documents for filing a claim before discharge from the hospital.
Claim forms
- Completed claim forms (PART I by you & PART II by attending doctor)
- Discharge Summary (if you stayed at a public hospital)

All receipts
- All receipts, for example:
- Medical receipt
- Deposit receipt
- Statement of charges
All medical reports & referral letters (if any)
- Treatment (physiotherapy, chiropractic, etc.)
- Surgery reports
- Laboratory reports (Blood test, urine test, etc.)
- Diagnostic imaging (MRI, CT scan, PET, etc.)

File the claim
Submit the claim application via FWD eServices app or online portal within 90 days from the treatment.

Select claim type

Follow the instructions to enter claim details

Upload documents

Review the claim details and click ‘Submit’
If your receipt amount exceeds HKD 50,000, you will be required to provide us with the original copies of the following documents:
- Claim form (PART I only)
- All original receipts
