The FWD Care recovery plan (‘the Plan’) is available for eligible customers with an approved claim via designated individual life, medical or critical illness insurance products on:
Designated Diseases mean any one of the following: Angioplasty of Coronary Artery, Benign Brain Tumour, Cancer, Carcinoma-in-situ, Cardiac Impairment Due to Primary Pulmonary Arterial Hypertension, Cardiomyopathy, Chronic Liver Disease, Coronary Artery Diseases Surgery, End Stage Lung Disease, Fulminant Hepatitis, Heart Attack, Heart Valve Surgery, Kidney Failure, Major Organ Transplantation, Parkinson’s Disease, Severe Rheumatoid Arthritis, Stroke, Surgery to Aorta (including Minimally Invasive Surgery to Aorta) or Terminal Illness.
Once your or your family member’s claim is approved, we’ll invite you or your family member to join the plan.
This Plan is not a part of the policy or benefit item under the policy provisions of designated individual life, medical or critical illness insurance products. The Plan is subject to terms and conditions. Please refer to the leaflet for details and full terms and conditions of this Plan, including definitions of Designated Diseases.
The Plan offers you one-on-one support from a registered nurse (‘Recovery Nurse’) with a range of support services. Upon the first phone conversation with the Recovery Nurse, if appropriate, the Recovery Nurse will arrange consultation with a clinical psychologist or cardiologist. After that your Recovery Nurse will discuss the support services based on your needs and the professional advices received from the clinical psychologist or cardiologist; book and organise these services on behalf.
Depending on the claim type, these are the support services that may be available to you or your family member:

The Plan does not constitute any legal advice and is not a substitute for legal advice. The legal consultation service is provided by designated third party service provider but not by FWD. Please consult your legal advisor’s independent advice on the appropriateness of any opinion to be provided to you. FWD shall not be responsible for or liable to the actions, negligence or omission of the designated third party service provider or the legal practitioner selected by you.
Also, FWD and the third party service provider reserve the right to terminate or vary the Plan in the sole discretion without further notice.

We’ll then assign a professional dedicated Recovery Nurse to you or your family member, as a companion throughout the recovery journey, no matter there are difficulties or challenges. Your Recovery Nurse will reach out as soon as we receive the consent from you or your family member to join the plan.
After the first telephone call with the Recovery Nurse, if appropriate, a caring session with a clinical psychologist or consultation with cardiologist will be arranged to assess your or your family member’s condition. Your Recovery Nurse will discuss the support services that best suit you or your family member’s needs and arrange the services on behalf.
